3. What happened then?

4. What has the reaction been?

5. How do you think it reached this point?

6. What else can you tell me?

7. What do you think contributed to this?

8. In retrospect, from your perspective—what could we have done to prevent this?

9. I'm sorry this happened. Is there anything we can do right now to help the situation?

10. This is extraordinarily important to me. How soon can we meet to discuss this in person?

11. Would it be helpful if I did some additional fact-finding and then we met tomorrow to discuss some proposed actions to address this?

12. If anything else surfaces in the meantime, can you let me know immediately?

13. Going forward, what would you think of trying to meet on a more regular basis? This will help us stay more aligned, reduce the chance of this ever happening again, and give us a chance to exchange perspectives on the work we're doing.
